Welcome to Hook Surgery. We are a small, friendly practice with a patient list of around 5000 in Chessington in Surrey. On this website you can book an appointment, order repeat prescriptions and view a summary of your patient record via EMIS Access (if you have signed up for it), as well as find information about who we are and the services we provide.

There is a car and cycle park behind the surgery accessed from Nigle Fisher Way.
We are on bus routes 465, 467 and K4 (Merritt Health Centre stop). The 71 also stops close by (Mansfield Road stop).
If you need to speak to a doctor out of hours when the surgery is closed, ring the usual number and you will automatically be put through to Kingston Health on Call.
Hook Surgery is a teaching practice. We feel we have a duty to make sure that there are good doctors and nurses for the future. We currently have links with St. George's Medical School, and teach medical students in the practice. We also sometimes have nursing students. If a student is present there will be a notice up on that day. If you wish to see the doctor or nurse without the student, please say so, the student will leave, and the consultation will go on as normal.
EMIS AccessWe are currently carrying out a survey through our Patient Reference Group to find out what you, our patients, think of the services we provide. To take part, please download the survey here. It can be filled in in Word by checking boxes and typing in text spaces. When you have completed it, please send it to kin-pct.hook@nhs.net. Alternatively, paper copies are available at the surgery. We will be accepting survey responses until January 27th.
