Online Services



Hook Surgery uses EMIS Access to make services available online to our patients. It can be used to book an appointment, order repeat prescriptions and view a summary of your patient record. To sign up you will need to fill in an application form (click here to download) and present it at the surgery along with some photo ID such as a passport or photo driving license. You will then be issued with a registration letter giving you all the security details you need. You must be aged 16 years or older to use this facility.
