Patient Groups

The Merritt Medical Centre Patient Participation Group (PPG) is formed of patients from Hook Surgery and our neighbours in Merritt Medical Centre, Chessington Park Surgery. The group is open to all patients registered at either surgery and exists to contribute to the services provided at Merritt Medical Centre from a patient-centred point of view. These can range from social and educational events to fundraising for equipment for the two surgeries to self-help groups.

Members of the group request and help organise educational events on a particular health topic that they feel is of interest to patients. Previous topics for events have included maintaining a healthy heart, diabetes, child safety, alcohol and drugs, COPD, staying healthy and safe (with the Chessington Safer Neighbourhood Team), men's health and smoking cessation.

In participation with the Patient Participation Group, we are currently setting up a Patient Reference Group to ask you, our patients, how we are doing and how you would like the surgery to develop. If you sign up we will occasionally send you a short survey by email (or by post if you do not have internet access) giving you the chance to have your say about the care and services you receive.

We would like to have a wide range of people answering to make the survey answers representative. So if you are a young person or of working-age or a retiree, white British or from an ethnic minority, a frequent visitor to the surgery or somebody we rarely see, we would like to hear from you.

To sign up, click on the links below either to fill in a form online or to download a printable form which can be returned to the surgery by hand or by post.

We are currently carrying out our first survey using a form which can be filled in in Word by checking boxes and typing in text spaces. It can be downloaded here. When you have completed it, please send it to kin-pct.hook@nhs.net. Alternatively, paper copies are available at the surgery. We will be accepting survey responses until January 27th.

Thank you for your help!
