Registering As A Patient

If you live within our practice area, roughly a 2 mile radius around the practice, you may register as a patient with us. If you are unsure if you fall in our area, please ask our receptionist.

You may attend the surgery with your medical card (issued by your previous doctor) or fill in a form which you can get from us.

You will be asked to make an appointment for a new patient check when you join so that we can get details of your past illnesses, family history and current medication. We will also check your blood pressure and test your urine, and may order other tests if appropriate.

People staying for less than three months in the area may register as a temporary resident if they are unwell, and can be seen by one of our doctors or nurses.

If you wish to register as a new patient, could you please fill in a New Patient Questionnaire before your New Patient appointment. It is available at the surgery or to download here. If you have not previously been registered at an NHS doctor could you please also fill in a GMS1 form which can be downloaded here.

Please note:

If you change your name, address, or telephone number, please make sure to inform us and include all members of your household in your details. There are times when we may need to contact you for examinations or results, and this is very difficult if we do not have up to date information.
